I have no idea. But you notice in the same pic, there's a halo around Dean / Jensen's head. It's obviously some peculiar symbolism that we're not privy to.
I don't know, to be honest. I *think* it's safe to watch again, Sam admitted to Dean at the end of the last episode that there's something wrong with him, which I take it to mean they'll now try and find an answer to the problem together, but you never know with SPN... Sam might just decide that being a better hunter this way is worth it and that he doesn't want any help from Dean figuring it out, that he wants to stay this way. I'm not entirely convinced yet...
It has been seen and mocked widely. The most common comments: have they even *seen* the show? And - why is Dean's head bigger than Sam's even though Sam is in the foreground?
Sigh. And they were doing so well with the grungy look TV promos.
